Passport Services


The Sublette County Library is a Passport Acceptance Facility. We will review your application and mail it to the Department of State for processing. Applications are mailed out the following business day. 

By appointment only.

You must apply in person using form DS-11 if at least one of the following is true:

  • You are applying for your first U.S. passport.
  • You are under age 16.
  • Your previous U.S. passport was issued when you were under age 16.
  • Your previous U.S. passport was lost, stolen or damaged.
  • Your previous U.S. passport was issued more than 15 years ago.

Applicants Under Age 16:

  • Both parents/guardians must authorize the issuance of the child’s passport.
  • Both child and parent/guardians must be present at the time of the appointment.
  • Please note that passports for children under 16 are only valid for 5 years.

Tracking Your Application:

Once your passport application has been accepted at the Library, all questions must be directed to the National Passport Information Center.

How to Apply:

You must fill out form DS-11: Application for a U.S. Passport prior to your appointment.

Online Form or Printable Form

How to complete your form:

  • Enter your information on the online form and print. This reduces possible errors caused by handwriting.
  • Print a DS-11 at the Pinedale Library and complete by hand.

Please note that DS-11 forms must be printed single sided.

If completing by hand, print legibly using black ink only.

Do not sign the DS-11 form until the passport acceptance agent instructs you to do so.

You must submit one of the following (original or certified copies only) as evidence of U.S. Citizenship:

  • Fully-valid, undamaged U.S. passport (may be expired)
  • U.S. birth certificate
  • Consular Report of Birth Abroad or Certification of Birth
  • Certificate of Naturalization
  • Certificate of Citizenship

Plan to present both a physical copy of a government-issued photo ID and a color paper copy at your appointment. The paper copy should be in color and show both sides of the ID.

You must provide one photograph with your application. To avoid processing delays, be sure your photo meets all photo requirements.

The Library is not performing photograph services at this time.

Two separate payments are required for a passport application:
  • Application Fee: Payable to the U.S. Department of State by check or money order. Click here to calculate your fee.
  • Processing Fee: A separate payment of $35 per application acceptable payment includes: check, money order, cash, or credit card.

To complete the application process, both payments must be processed at the time of your appointment.

Passport Appointments

New Passports:

If English is not your first language, we kindly request that you bring a translator to ensure the most productive appointment experience.

Passport Renewals:

Adults who hold a 10 year passport, issued less than 15 years ago must send their applications directly to the U.S. Department of State

Our office does not process Adult Renewal Applications. However, we are happy to look over your documents. Call the Pinedale Library at (307) 367-4114 to set up a passport renewal appointment.
